All praise is due to Allah, and peace and blessings be upon our master, the Messenger of Allah.
Saying the basmalah (Bismillāh) at the beginning of wuḍūʾ is a sunnah. It is stated in al-Minhāj al-Qawīm Sharḥ al-Muqaddimah al-Ḥaḍramiyyah (p. 27): “Its sunnahs include the siwāk, then the tasmiyah (saying Bismillāh), joined with the intention (niyyah), at the start of washing the hands.” And Allah Almighty knows best.

All perfect praise be to Allah, The Lord of The Worlds                                                                                                                                                                  Yes, saying Tasbeeh once during Rukoo` and Sujood is enough, provided that the worshiper`s attentiveness of the heart isn`t undermined during each. All perfect praise be to Allah, The Lord of The Worlds.                                                                                                                                                                  Any other part of Qur'an that a worshiper reads after Al-Faatihah during the first and the second Rak`ahs is a Sunnah, and forgetting to do so doesn`t nullify prayer, and doesn`t require performing Sujood As-Sahw (prostration of forgetfulness). And Allah Knows Best.
